Abstract (English): BACKGROUND: Postpartum hemorrhage remains a leading cause of maternal mortality worldwide. In Mexico, postpartum hemorrhage was the leading cause of direct maternal death in 2023. OBJECTIVE: To review the medical care of patients with postpartum hemorrhage and new technologies such as the JADA system. METHODOLOGY: A semi-systematic review that included articles in English and Spanish of the type of controlled clinical trials, cohort and observational studies with a publication date in the last 11 years (2014 to 2024). The reference search was limited to the PubMed-Medline database, with the search strategy defined by the keywords (MeSH): "hemorrhage/prevention & control", "hemorrhage/therapy". RESULTS: 1083 articles were identified. In the first stage of the selection process (title and abstract), 300 articles were retained. In the second phase (full text), 35 were selected and the rest were rejected for not being directly related to postpartum hemorrhage, duplicate articles, studies with weak methodology, or not relevant. CONCLUSIONS: The choice of treatment depends on the severity and cause of the bleeding, the patient's response to initial general treatment, and the availability of resources and specialized medical equipment. It may include a combination of noninvasive or invasive measures, ranging from homeostatic treatment to surgical intervention. Evidence has shown that the intrauterine vacuum-controlled hemorrhage device has the potential to be used early to rapidly and effectively control postpartum hemorrhage. [ABSTRACT FROM AUTHOR]
Abstract (Spanish): ANTECEDENTES: La hemorragia posparto sigue siendo una de las principales causas de mortalidad materna en todo el mundo. En México, para el año 2023, la hemorragia obstétrica fue la primera causa de muerte materna directa. OBJETIVO: Revisar la atención médica a la paciente con hemorragia posparto y las nuevas tecnologías, como el dispositivo intrauterino de control de hemorragia por vacío (JADA). METODOLOGÍA: Revisión semisistemática que incluyó artículos en inglés y español, del tipo de ensayos clínicos controlados, estudios de cohorte y observacionales, con fecha de publicación de los últimos 11 años (2014 a 2024). La búsqueda de referencias estuvo limitada a la base de datos PubMed-Medline con la estrategia de búsqueda definida por las palabras clave (MeSH): "Hemorrhage/prevention & control", "Hemorrhage/therapy". RESULTADOS: Se encontraron 1083 artículos. En la primera fase del proceso de selección (título y resumen) se conservaron 300 artículos. En la segunda fase (texto completo) se seleccionaron 35 y el resto se rechazó por no estar directamente relacionados con la hemorragia posparto, artículos duplicados, estudios con metodología débil o no relevantes. CONCLUSIONES: La elección del tratamiento dependerá de la gravedad y la causa de la hemorragia, la respuesta de la paciente al tratamiento general inicial y la disponibilidad de recursos y equipo médico especializado. Puede incluir una combinación de medidas no invasivas o invasivas que pueden ir desde un tratamiento homeostático hasta intervenciones quirúrgicas. La evidencia ha demostrado que el dispositivo intrauterino de control de hemorragia por vacío puede utilizarse de manera temprana para controlar rápida y eficazmente la hemorragia posparto.
