Abstract (English): Forty two Coventry high tibial osteotomies (HTO) using conventional staples and cast immobilization make in patients with osteoarthrosis of the medial compartment of the knee, were compared to forty five using cast immobilization alone without staple and neither internal fixation; the median follow-up was 7.2 years. The data was subjected to student's test. At the final follow-up examination, in the group of the forty two osteotomies with staple and cast, thirty two knees (76%) improved and pain at rest was absent and mild to walking long distances (VAS). In the group of the forty five osteotomies with cast immobilization alone, thirty five knees (78%) were pain relief at rest and mild to walking long distances (VAS). These results suggest that high HTO can be performed without using Coventry staple and neither other internal fixation. [ABSTRACT FROM AUTHOR]
Abstract (Romanian): Para evaluar si en la estabilización de la osteotomía valguizante proximal de tibia tipo Coventry, es indispensable la utilización de las grapas escalonadas, se efectuó estudio retrospectivo en 87 rodillas con tibia vara y gonartrosis de compartimiento medial a quienes se les efectuó esta técnica. En un grupo de 42 rodillas, se colocaron grapas y aparato de yeso. En otro grupo de 45 rodillas sólo se colocó aparato de yeso sin utilizar la grapa. En un seguimiento promedio de 7.2 años, en el grupo de las 42 rodillas manejadas con grapa-yeso, treinta y dos (76%) manifestaron dolor ocasional en la escala visual análoga del 1 al 3, cinco rodillas (12%) tenían dolor severo de 5 a 8 (EVA) y cinco rodillas (12%) habían sido reintervenidas para artroplastía total. En el grupo de las 45 rodillas manejadas con aparato de yeso sin grapa, treinta y cinco (78%) referían dolor ocasional en la escala visual análoga del 1 al 3, cuatro rodillas (9%) tenían dolor severo y seis rodillas (13%) habían sido reintervenidas para artroplastía total de rodilla al final del seguimiento. Se concluye que las grapas de Coventry no son indispensables en la osteotomía tibial valguizante, ya que en el seguimiento a 7.2 años, no se observó diferencia significativa en la evolución clínica ni radiográfica, entre el grupo al que se les colocó grapa y aparato de yeso y el otro grupo al que únicamente se les colocó aparato de yeso (P > 0.005).
