Abstract (English): In this article, the authors highlight that, contrary to what has been believed until recently, the strictly vegetarian population whose diet is based on plants (vegetables, greens, whole grains, legumes, fruits, seeds, and seaweed), typical of temperate or mediterranean climate, does not suffer from vitamin B12 deficiency. They propose that, similar to herbivorous animals, vitamin B12 is mainly provided by bacterial biosynthesis occurring in the small intestine, especially in the ileum, thanks to acidophilic microbiota (bifidobacteria and lactobacilli) that colonize the small intestine early in life. This function is supported by prolonged breastfeeding, emphasizing the importance of the bifidus factor that promotes the growth of bifidobacteria. It is significant that vitamin B12 absorption occurs precisely in the terminal ileum. In other words, in strict vegetarians, the vitamin is absorbed in the same place where it is produced. Abstract (Spanish): En este artículo, los autores ponen en evidencia que, contrariamente a lo sostenido hasta fecha reciente, la población vegetariana estricta, con una dieta basada en plantas (verduras, hortalizas, cereales y granos integrales, leguminosas, frutas, semillas y algas marinas), propia de los países de clima templado o mediterráneo, no adolecen de deficiencia de vitamina B12. Postulan que, al igual que en los animales herbívoros, ésta es provista principalmente por biosíntesis bacteriana, que se desarrolla en el intestino delgado, especialmente en el íleon por la microbiota acidófila (bífidobacterias y lactobacilus), que colonizan tempranamente el intestino delgado, función favorecida por la lactancia materna prolongada, destacando la importancia del factor bífidus que promueve el crecimiento de las bífidobacterias. Es significativo que la absorción de la vitamina B12 ocurre precisamente en el íleon terminal. Vale decir, en los vegetarianos estrictos, la vitamina se absorbe en el lugar donde se produce.
