Abstract (English): Ischial osteomyelitis is a bone infection that is very infrequent during childhood and is diagnosed by excluding other more frequent conditions. The definitive diagnosis is made with puncture biopsy, which allows instituting targeted antibiotic therapy. We present herein two clinical cases of two children who had a favorable course. In both cases the fi ndings of the initial X-rays were unremarkable. The physical exam did not show total limitation of the coxofemoral joint but it did show ischial tenderness. The MRI showed images compatible with ischial osteitis. The ischial puncture biopsy was diagnostic in both cases. At the 24-month follow-up both children are leading a normal life. Conclusions: Ischial osteomyelitis is an entity to consider in children with coxalgia once other more frequent conditions have been ruled out. Abstract (Spanish): La osteomielitis isquiática es una infección ósea muy infrecuente en la edad infantil que se diagnostica por exclusión de cuadros más frecuentes. El diagnóstico definitivo se obtiene mediante punción-biopsia, la cual permite establecer un tratamiento antibiótico dirigido. Presentamos dos casos clínicos de dos niños con evolución satisfactoria. En los dos casos la radiología inicial fue anodina. La exploración clínica no mostraba total limitación de la articulación coxofemoral pero sí dolor a la palpación isquiática. En la resonancia se aprecian imágenes compatibles con osteítis isquiática. La punción biopsia isquiática fue diagnóstica en los dos casos. A los 24 meses de seguimiento ambos niños realizan una vida normal. Conclusiones: La osteomielitis isquiática es una entidad a tener en cuenta en niños con coxalgia, tras la exclusión de patologías más frecuentes.
