Abstract: This article explores various dimensions of the case presentation, especially connections with Searles' extensive interviews with Robert Langs, which provide much relevant personal and professional background for his work with Joan Douglas. Since Searles told me in 2000 that he believed some of his previous patients actually had multiple personality, I explore what Searles once called multiple identity functioning in Joan. I also respond to selected topics presented in the other commentary articles in this issue. I challenge the current belief that medications are the most important treatments for severe mental illness, and I suggest that what we can learn about treatment from this 1961 case conference remains relevant today, and psychodynamic treatment approaches for severely ill patients should not be abandoned.
