Abstract (English): Heart failure (HF), is a clinical syndrome characterized by signs and symptoms of veno-pulmonary congestion and cardiac dysfunction. Heart transplant is the treatment of choice for decompensated heart failure without response to medical treatment. The highlights in the care of post-transplant patients consider hemodynamic, ventilatory, neurological, and renal management, graft ischemia time, and histological severity of rejection, among others. The Nursing Subdirectorate of the HGMEL implemented a training program for the nursing staff of two Intensive Care Units called "Essential Post-Surgical Nursing Interventions in the Patient Undergoing a Heart Transplant" and an administrative instrument called "Guide for Nursing Care of the Patient" was created. Subjected to Heart Transplantation in the Postoperative Period" which contains the primary activities that correspond to the scope of responsibility of the nursing staff at the different hierarchical levels and has the purpose of serving as a reference to contribute to the safety of the patient with a heart transplant, in the postoperative period. that in the event of an event that is not common in the daily life of the institution. Abstract (Spanish): La insuficiencia cardiaca (IC), es un síndrome clínico caracterizado por signos y síntomas de congestión veno-pulmonar y disfunción cardiaca. El trasplante cardiaco es el tratamiento de elección de la insuficiencia cardiaca descompensada sin respuesta al tratamiento médico. Los aspectos destacados en los cuidados a los pacientes posoperados de trasplante consideran el manejo hemodinámico, ventilatorio, neurológico, renal, el tiempo de isquemia del injerto, la gravedad histológica del rechazo, entre otras. La Subdirección de Enfermería del HGMEL implementó un programa de capacitación al personal de Enfermería de dos Terapias Intensivas llamado "Intervenciones de Enfermería Posquirúrgicas Esenciales en el Paciente Sometido a Trasplante Cardiaco" y se creó un instrumento administrativo denominado "Guía para el Cuidado de Enfermería del Paciente Sometido a Trasplante Cardiaco en el Posoperatorio" que contiene las actividades primordiales que corresponden al ámbito de responsabilidad del personal de Enfermería de los diferentes niveles jerárquicos y tiene la finalidad de servir como referente para contribuir a la seguridad del paciente con trasplante cardiaco, en el postoperatorio que ante un evento que no es común en la vida cotidiana de la institución.
