Abstract (English): In neuropsychological diagnosis, there are two approaches that can be characterized as psychometric neuropsychology and qualitative neuropsychology, depending on the emphasis each one places on how difficulties are assessed during task performance. Psychometric neuropsychology focuses on quantifying correct or incorrect responses, and in some cases, a small numerical scale is assigned to performance (0, 1, 2). In qualitative neuropsychology, the emphasis is on analyzing the nature of errors and identifying under what conditions of activity regulation task performance improves or worsens. Qualitative analysis should be understood as a flexible and systematic process that allows for the diagnosis of alterations in the organization of psychological activity and provides insight for its reorganization. The objective of this study is to describe the syndromic analysis process in a case of ischemic stroke whose severity made it impossible to use psychometric tests or standardized protocols. A single-case observational design was used. The results made it possible to identify the syndrome of sequential motor organization, according to the model of A.R. Luria. The systemic effect of the primary defect on the patient's verbal activity is presented. The study concludes that qualitative analysis of signs and symptoms leads to an accurate diagnosis, thus laying the groundwork for the reorganization of the impaired functional systems. Abstract (Spanish): En el diagnóstico neuropsicológico existen dos aproximaciones que podemos caracterizar como neuropsicología psicométrica y neuropsicología cualitativa, a partir del énfasis que cada una de ellas otorga a la forma de evaluar las dificultades que se evidencian en la ejecución de diferentes tareas. En la neuropsicología psicométrica, el énfasis se centra en cuantificar las respuestas correctas o incorrectas, y en algunos casos se asigna una pequeña escala numérica a la ejecución (0, 1, 2). En la neuropsicología cualitativa, el énfasis se centra en analizar la naturaleza de los errores y bajo qué condiciones de regulación de la actividad mejora o empeora la ejecución de la misma. El análisis cualitativo debe entenderse como experimentos flexibles y sistemáticos que permiten diagnosticar las alteraciones de las formas de organización de la actividad psicológica y dar pistas para su reorganización. El objetivo del presente estudio es describir el proceso de análisis sindrómico en un caso de EVC isquémico cuya gravedad imposibilitó el uso de test psicométricos o protocolos estandarizados. Se empleó un diseño observacional de caso único. Los resultados permitieron identificar el síndrome de organización secuencial motora de acuerdo al modelo de A.R. Luria. Se presenta el efecto sistémico del defecto primario en la actividad verbal del paciente. Se concluye que el análisis cualitativo de los signos y síntomas conducen al diagnóstico preciso, generando así las bases para la reorganización de los sistemas funcionales dañados. Abstract (Portuguese): No diagnóstico neuropsicológico, existem duas abordagens que podem ser caracterizadas como neuropsicologia psicométrica e neuropsicologia qualitativa, com base na ênfase que cada uma coloca na avaliação das dificuldades evidentes na execução de diferentes tarefas. Na neuropsicologia psicométrica, a ênfase está na quantificação de respostas corretas ou incorretas e, em alguns casos, uma pequena escala numérica é atribuída ao desempenho (0, 1, 2). Na neuropsicologia qualitativa, a ênfase está na análise da natureza dos erros e em quais condições de regulação da atividade o desempenho melhora ou piora. A análise qualitativa deve ser entendida como experimentos flexíveis e sistemáticos que permitem o diagnóstico de alterações nas formas organizacionais da atividade psicológica e fornecem pistas para sua reorganização. O objetivo deste estudo é descrever o processo de análise sindrômica em um caso de acidente vascular cerebral isquêmico cuja gravidade impediu o uso de testes psicométricos ou protocolos padronizados. Foi utilizado um delineamento observacional de caso único. Os resultados permitiram identificar a síndrome da organização motora sequencial segundo o modelo de A.R. Luria. O efeito sistêmico do defeito primário na atividade verbal do paciente é apresentado.
