Abstract (English): Prior to the onset of immunoglobulin antiD, many of the fetuses of mothers negative for the antigen “D" developing severe disease, history of prenatal diagnosis of alloimmunization is the perfect example of constructive effort by a diagnosis in order to identify cases in need of therapy to decrease morbidity and increase survival with the least number of invasive procedures and reducing the risks associated with them. Today it is difficult to determine the true prevalence of the disease in our environment, but the understanding of the pathophysiology has helped the evolution of diagnostic tests and better treatment approach to positively impact the evolution of the disease. Abstract (Spanish): Antes de la aparición de la inmunoglobulina antiD muchos fetos de madres negativas para el antígeno “D" resultaban con enfermedad severa. La historia del diagnóstico prenatal de aloinmunización es el perfecto ejemplo del esfuerzo por un diagnóstico constructivo con la finalidad de identificar los casos que requieren tratamiento para disminuir la morbilidad e incrementar la supervivencia mediante el menor número de procedimientos invasivos y reducir sus riesgos asociados. En la actualidad es difícil determinar la prevalencia real de la enfermedad; sin embargo, el entendimiento de la fisiopatología ha ayudado a la evolución de las pruebas diagnósticas y a un mejor enfoque de tratamiento, con repercusión positiva en la evolución de la enfermedad.
