Abstract: The author reflects on the long term costs and outcomes of hydromorphone therapy or diacetylmorphine therapy to treat severe opioid use disorder compared to methadone therapy. He states over the long term there is lower costs and better outcomes for both injectable treatments than methadone maintenance. He mentions that reduced criminal activity is responsible for much of the cost savings from injectable therapy and talks about how stigma is the chief barrier to injectable opioid therapy.
