The article discusses the case of an older adult which demonstrated the use of home-based primary care (HBPC) in reducing medication-related harm. Topics discussed include the side effects of polypharmacy, the education of caregivers to reduce medication errors and improve the patient's outcome, and home visits for close medication titration and monitoring. |
